Annual fundraising event supporting Easterseals NH programs and services — a 5K walk/run through historic downtown Manchester finishing at Veteran’s Memorial Park with family-friendly festivities and a 90th birthday “human 90” celebration.

Catering & aid stations

On-course refreshments and water stations provided; food vendors may be available at the event.

Pacers

Pacers will lead target times per start wave listed on the event schedule (Renntag) at Veterans Memorial Park; expect pacers on course with flags to help runners maintain goal pace.

What is Easterseals National?

Who is Eligible?

New Hampshire families with a child (birth to age 21) suffering from a chronic health condition or developmental disability that is expected to last 12 months or longer and: Affects the child’s ability to function daily and/or Requires frequent and intensive medical care Does My Child’s Condition Qualify? Easterseals NH Health Care Coordination assists children and young adults suffering from: Severe chronic conditions such as organ failure Moderate conditions like diabetes Mild conditions like asthma Clients with developmental disabilities may also be eligible

How We Can Help

Health Care Coordination through Easterseals NH helps families identify their child’s needs. We work directly with families to locate and access resources and support systems. Our care coordinators collaborate with schools, insurance providers, and healthcare professionals to ensure optimal care. We offer recreational opportunities and encourage social interaction between families facing similar challenges. Additionally, we provide respite for caregivers and financial assistance when necessary. Our program fosters a strong community support network for the families we serve. Our Goal Easterseals NH Health Care Coordination aims to empower those we serve to handle chronic illness independently. We provide the experience that strong families need to advocate for themselves and the knowledge required to locate and utilize community resources to build their support system.

Convenience – Choice of therapy locations that work for your child and your family Affordability – Free evaluation at home or in the clinic. Free physical therapy, occupational therapy, speech therapy, and specialized instruction for eligible children. There is never an out-of-pocket cost to families. Service Coordination – We coordinate services with pediatricians, schools, and other providers High-Quality, Evidence-Based Care – Our staff is licensed, credentialed pediatric professionals. They are certified in specialized techniques not widely found, including DIR Floortime, select mutism, and more. School Transition Services – We help your child make the transition to preschool or other services at age 3 Ongoing Assessments – We track and provide updates on your child’s development Advocacy Training – We give you tools and training to help you work with your child’s schools, therapists, doctors, and community groups Direct Services in English and Spanish Family Resources and Support – Families learn how to incorporate therapy into their child’s daily activities Access to Additional Children’s Services through Easterseals NH & VT, including Autism Services, Pediatric Outpatient Services, and inclusive Child Care.

Our team of highly-qualified professionals has extensive experience supporting children with various conditions, including torticollis, down syndrome, Turner syndrome, feeding syndromes, hearing and vision disabilities, autism, and NAS (infants born with substance use addiction). Who We Serve

Children may be eligible for Easterseals NH FCESS at no cost if they are: Between the ages of 0 and 3 years old Have a developmental delay, an established condition, OR are at substantial risk for a developmental delay Live in our area of service (Greater Manchester or Greater Salem) An ESS team will provide a Developmental Evaluation to determine your child’s eligibility according to federal and state criteria. For eligible children, the parents and the ESS team will develop an Individual Family Support Plan (IFSP) that includes family and child outcomes and strategies. The appropriate professional will initiate services. Easterseals FCESS serves the following towns in Southern New Hampshire. If you do not see yours on the list, visit the New Hampshire Department of Health & Human Services (NHDHHS) to find your local Family Centered Early Supports and Services provider. What if my child is over three?

Family Centered Easterseals Early Supports and Services (FCESS) is for children ages zero to three. If your child is three or older, please explore our Pediatric Outpatient Services or our Autism Services program.

How much do your services cost?

Easterseals NH FCESS is provided under contract with Moore Center Services, Inc. Services are funded by private insurance, Medicaid, state and federal programs, and ESNH fundraising. There is never an out-of-pocket cost to families.

How do I schedule a free evaluation?

To determine eligibility for therapeutic services for children between the ages of 0 and 3, living in the Manchester area, please contact the Moore Center at 603.206.2700 to schedule a free evaluation.

How long does it take to start services?

We provide a service plan for eligible children within 45 days of intake.

What community resources do you offer?
